A.C. Fosdick, a doctor in Liberty, Union County, Indiana, was commissioned on October 18, 1862 as surgeon of the Fifth Cavalry Ninetieth Indiana regiment. During Fosdick's service, the regiment was stationed primarily in Louisville, Munfordsville and Glasgow, Kentucky. On October 5, 1863, Fosdick resigned. He later returned to his practice in Liberty.

The collection, spanning the years 1838 to 1905, contains personal and military papers of A.C. Fosdick. The first folder consists of medical receipts for various diseases and ailments (n.d.); election notices from Boston, Indiana (1847, 1848); a state election notice (1852); a turnpike directors meeting cancellation notice (1854); a medical waver (1862); and a newspaper clipping of the Early History of Hanna's Creek Christian Church (1905). In addition, a travel diary (1838) written on a camping trip in east central Indiana does not include the author's name. A letter (1859) to John Allen from an unknown person in Stockdale, Indiana describes a trip to "bleeding" Kansas. A second letter (1859) from James Heslep to an unknown recipient is also included. Finally, four circular orders from the Medical Director's Office (1863) outline bureaucratic medical procedure to surgeons.
Folder two contains Fosdick's resignation papers (November 1863) and various Surgeon's Reports (1863). Also included are special orders (1863) demanding Fosdick's service at numerous places and giving notice of an upcoming examination. Folder three contains a requisition and several invoices for medical supplies (1863), and a discharged soldiers list (n.d.). A hospital fund notebook (n.d.) and a letter (1876) to Fosdick from Henry J. Johnson, Attorney, involving the wording of an affidavit are also included. OM 275 contains a requisition (April 1863) and several invoices for medical and hospital supplies (June - July 1863), and a report of sick and wounded (July 1863).
